Pleasant Elementary Kindergarten Screening

for 2020-2021 School Year

 

Who: Children who will be five years old by August 1, 2020     

What: Kindergarten Registration and Screening 2020-2021 school year         

Where: Pleasant Elementary---1105 Owens Rd W (Gym & Cafeteria)

When: Thursday, August 13th

Time Slots: 9am, 10am, 11am, 1pm, 2pm, 3pm, 4pm

 

*ODE requires all children entering kindergarten be screened for hearing, vision, speech and communications, health or medical problems, and for any developmental disorders.  This will allow school personnel to work with parents to get any assistance the child may need. 

 

Prior to your arrival on screening day, you will need to do the following:

 

  1. Register at:https://pleasantmarion-oh.finalforms.com/ and follow the prompts to create your account, create your student, and sign registration forms.
  2. Upload birth certificate, shot records, and proof of address.
  3. Go to https://www.signupgenius.com/go/8050f4fa8ac29a7fc1-20202021 and choose your time slot for screening.

 

IMMUNIZATIONS

Minimum immunization requirements are Hepatitis B Series, 5 DPT(A fifth dose is required if the fourth dose was administered prior to the 4th birthday), 4 Polio (A fourth dose is required if the third dose was received prior to the 4th birthday), 2 MMR (Measles, Mumps, Rubella) and the 2 Varicella (the Chickenpox Vaccine).  Please update your records before registration.  The State Health Department is concerned about children not immunized.  Any pupil who does not meet the above stated criteria is in violation of section 3313.673 (and can be excluded from school).  

 

SCREENING

The screening gives the school an opportunity to measure a child’s readiness.  Screening will include speech, hearing, vision, health, medical, developmental (physical and mental), and parent/student interviews.  Recommendations will be given to parents if there are problems with any portion of the screening.

 

OPEN ENROLLMENT:

If you do not live in the Pleasant school district we do accept open enrollment.  You will need to follow the steps listed above as well as visit our Administrative Offices located at 1107 Owens Rd. W, Marion Ohio 43302.  Please complete all other aspects of registration prior to open enrollment.
